Affordable Care Act Enrollment

Trained representatives from Horizon Blue Cross Blue Shield will be available to assist people with enrolling in the Health Insurance Marketplace.

What to Bring to Your Appointment:

• Proof of Lawful Residency (Green Card) or Work Authorization (H1B Visa)
• Social Security Number
• Proof of State Residency (Rental Receipt)
• Proof of Monthly Household Income (pay stubs, unemployment payment stubs, or W-2)
• Copy of the previous year tax returns

If you are currently enrolled in a Health Insurance Plan, please bring the policy information or renewal notice for that plan.
