If you attend Churchill Junior High School, your packet is here. If you attend EBHS, your packet is here. Both packets must be filled out the same way.
When you start your volunteering at EBPL, fill out the request for approval and give just that page to your guidance counselor or grade level administrator. The library is already pre-approved at both schools, so you don't need to wait for approval, just give them that page so they know where you're doing your service and then start signing up for volunteer hours. Fill out the request for approval as follows:

Throughout the year, keep your time sheet up to date. Every time you complete a volunteer shift at the library, fill out the Google Form at the library and also write it on your paper time sheet when you get home. If you forget to update your paper time sheet and have lost track of your hours, email teens@ebpl.org and we can send you a copy of all your responses from the Google Form. For this reason, the Google Form from the QR code at the library should always be your first priority! If you forget to fill out the paper time sheet, we can give you your information from the Google Form, but we cannot use your paper time sheet to add them to the Google Form!
In April, or whenever you reach 15 hours of service at the library, fill out the top half of the evaluation form (name, student #, etc.) and your entire time sheet, and hand it in at the library. You must leave it with a staff member, do not just drop it on an empty desk! If no one is at the teen desk, give it to a staff member at the Youth Services desk. School packets must be dropped off at the library no later than May 1st! We have more than 200 teen volunteers who need their forms filled out. If you drop off your packet after this deadline, we cannot guarantee it will be ready before the school deadline for submission, which means you may need to request an extension. As long as you have filled out the top half of the form with your full name, we will email you to let you know when your packet is ready to be picked up. You can do this any time the library is open by giving your full name to a staff member at the Youth Services desk. For both packets, fill out the organization info as follows:
-
Date service began: (list the date of your first volunteer shift for the school year)
-
Date service ended: Ongoing
-
Name of Placement: East Brunswick Public Library
-
Sponsor Name: Youth Services Department
If you volunteered at multiple organizations, you must fill out one packet for EACH location! Ex. Aanya completed 7 hours through Youth Council and 12 hours through the library, so she submits one packet filled out by a Youth Council advisor (with just the 7 hours from Youth Council) and one packet filled out by a library staff member (with just the 12 hours from the library). As long as the total number of hours from both organizations is 15 or higher, you will get credit on your transcript for completing your volunteer service requirement for that year.
Before you give your completed packet to school, you must also write a brief essay. The library does not need your essay included in your packet; this information is purely to assist you in preparing your packet to be handed in at school. These essay are not graded and do not need to be extremely well-crafted. All you have to do is talk about what you did during your volunteering service and how you grew as a person from it. Ex: 3 paragraphs talking about how you learned responsibility by helping out with a children's origami program.
Once your evaluation and time sheet are signed by a library staff member and your essay is complete, printed, and attached to your packet, you are ready to submit it to school! For 8th and 9th graders at Churchill Junior High School, submit your packet to the Counseling Office secretary. For 10-12th graders at EBHS, submit it to your Grade Level Administrator through their secretary.
Packets are generally due around May 12-15 every school year. Please check with your guidance counselor or grade level administrator to verify the due date for this school year!
